Welcome to the website of Platinum Psychiatry LLC (“we,” “us,” “our,” or “Platinum Psychiatry”), located at www.platinumpsychiatry.com (the “Site”). These Terms and Conditions of Use (“Terms of Use”) govern your use of our Site and our Services. By using our services or making payment, you affirm your acceptance of these Terms of Use and acknowledge our Privacy Policy. If you do not agree, you may not access or use our Services.
“You” and “your” refer to the user of the Services, including dependents, anyone accessing the User Account, and legal successors.
Patient/Provider Responsibilities
Patients must provide accurate contact and billing information and inform us of changes promptly. Platinum Psychiatry provides outpatient psychiatric services, which may not constitute comprehensive psychiatric care. Patients are responsible for pursuing any additional care needed. Psychiatry does not replace primary medical care.
Description of Services
Our Services may include:
Self-assessments for mental health conditions.
Access to educational content on behavioral health.
Connection with licensed Healthcare Providers.
Counseling, psychiatric care, and medication management.
Ongoing care monitoring and mental health resources.
Subscription Terms
We offer clinical and non-clinical services via telehealth through subscription plans. These may include counseling, diagnostics, medication management, care monitoring, and educational resources. Subscriptions auto-renew and are subject to change.
Membership includes one session per month if clinically indicated.
Additional non-clinically indicated sessions are $89 each.
Subscription cancellation requires 30 days’ notice.
Attendance Policy
Patients receiving medications must attend appointments regularly. For controlled substances (CII), appointments are required every three months. Failure to attend may prevent medication refills. Cancellations require 24-hour notice, or a fee of $89 will be charged.
Prescription and Pharmacy Services
Prescriptions are issued after provider consultations. Platinum Psychiatry does not endorse any specific pharmacy or medication. Patients must follow all prescribed medication guidelines. Prescription requests must allow at least 48 hours for processing.
Telehealth Services
Services are delivered electronically. Telehealth is not a replacement for in-person healthcare. Patients must review and accept the Telehealth Informed Consent before receiving services.
Privacy Practices
Use of Services indicates acknowledgment and receipt of our Notice of Privacy Practices detailing how we collect and use health information.
Emergency Disclaimer
Our Services are not suitable for emergencies. Call 911 for immediate medical help or 988 for mental health emergencies.
Account Management
Users must create and maintain one accurate, non-transferable account. Users under 18 require guardian supervision. We may deactivate accounts for fraud or misconduct.
Payment and Billing
Payments are processed through your saved credit/debit card. Subscriptions are billed automatically. Additional fees (e.g., no-show) are charged separately and due promptly.
Refunds
All fees are non-refundable unless required by law. You may cancel subscriptions to prevent future charges. Refunds or credits are at our discretion.
Laboratory Services
Lab tests require valid prescriptions. Lab costs are billed separately. Failure to complete required tests may affect your treatment.
Compliance with Rules and Prohibitions
Users must follow platform rules. Prohibited activities include:
Impersonation or identity misrepresentation.
Creating multiple accounts for deception.
Allowing others to access your account.
Harassment or disruption of providers or patients.
Recording sessions without consent.
Seeking prescriptions from multiple providers (doctor shopping).
Falsifying message origins.
Violating others’ privacy.
Promoting ads or content without permission.
Misusing platform functionality.
Violation may result in account suspension or termination without notice.
Health Information Exchange (HIE/HIN)
We may participate in health information networks for coordinated care. You may opt out by emailing compliance@platinumpsychiatry.com.
Changes to Terms
We may update these Terms at any time and will notify you of significant changes. Continued use of Services constitutes acceptance of updates.
Eligibility
Services are for users 18+ or minors aged 13–17 with guardian consent.
Feedback
You may submit suggestions to office@platinumpsychiatry.com. We may use this feedback without obligation.
Third-Party Services
We may refer you to third-party services. We are not responsible for your interactions with those services.
Termination
You or Platinum Psychiatry may terminate your subscription at any time. We may deactivate your account for violations or misuse.
Disclaimers
Services are provided “as is.” We do not guarantee outcomes and do not substitute emergency care.
Limitation of Liability
Our liability is limited to $100 or the amount you paid in the last five months, whichever is greater.
Governing Law
These Terms are governed by Arizona law. Legal disputes must be resolved in Maricopa County, Arizona.
Dispute Resolution
All disputes must be resolved via binding arbitration in Maricopa County, Arizona. Class actions are not permitted.
Communications and Notices
By using our Services, you consent to receive communications via email, text, or phone. Standard carrier rates may apply. You may opt out at any time.
